The Client understands and agrees as follows:
- All forms required by KILOS Performance and Fitness LLC, including health history, liability waiver and release, and if applicable, physical therapy guidelines, and/or physician’s approval, must be completed and/or signed prior to commencement of the training program.
- Training sessions will begin promptly at the time agreed upon by the Client and trainer, and not at the time of the Client’s arrival. Services will incur charges from that the agreed-upon start time forward. Please be on time in order to make full use of the training session.
- If a group training individual shows up 15 min or more late they are removed from that training time. If people show up late and are not done their training by the end of their scheduled training hour, they no longer receive coaching.
- Twenty-four hour notice is required for cancellation of a training session. Cancellation occurring less than twenty-four hours in advance may incur a one-hour no-show fee.
- Payment method for training may be by check or credit card. The payment for all sessions must be paid in full prior to or following the first training session.
- Client shall be responsible for any charges incurred by KILOS Performance and Fitness LLC in the collection of fees due from Client.
- Rates are set for the initial training period, but are subject to change thereafter, as may be set forth on a new Training Package sheet, which shall be deemed incorporated into this Agreement. All subsequent training sessions remain subject to all other terms of this Agreement.
- Client will engage in appropriate pre-exercise warm-up and post exercise cool down, stretching and flexibility exercises.
- Client will use exercise equipment, perform flexibility exercises, and perform muscular strength or muscular endurance exercises, and aerobic activities only in a manner instructed.
- Client will perform activities at intensity level appropriate for Client’s general health and physical condition.
- Client will immediately cease activity if Client feels any physical symptoms that are unusual for the Client or if indicated by the personal trainer.
- Client shall not to participate in activities while under the use of alcohol, drugs, or prescribed medication unless specifically approved by Client’s physician.
- Client understands that Client has control of the personal training session, and may end a particular exercise or workout at any time, and agrees to notify the personal trainer of any changes in physical condition before the start of or during a training session, including fatigue, shortness of breath or chest pains.
- Client understands that the results achieved from personal training are the result of a combination of exercise, nutrition and Client’s personal genetics, and KILOS Performance and Fitness LLC does not make any representation or guaranty of results.
- Client understands that Client’s personal training is not a doctor, physiotherapist or dietician, and that the advice offered is option and not a replacement for professional advice from experts within specialist fields.
